Nasal Surgery (Rhinoplasty)
   

Each year, hundreds of people seek cosmetic enhancement of their nose.  Since it is the most prominent feature on the face and so essential to facial symmetry, the nose is the most defining characteristic of the entire face.  Not only is it intricately connected to one’s looks but also to one’s heritage, ethnicity, facial character and strength.  Whether a person wants to improve upon the nose that nature dealt, to correct a nasal injury or deformity, or to change the effects of aging on the nose, individuals who undergo rhinoplasty usually find that even a slight alteration can greatly improve their appearance. 

Rhinoplasty means “shaping the nose.”  The majority, if not all, of the stitches are dissolvable and made inside the nose, where they do not show.  Through small incisions, the bone and cartilage are sculpted to provide a new structure.  In other words, the cartilage may be reduced, enhanced, or simply reshaped.  The tip may be elevated, the base may be narrowed, using tiny stitches inside the nostrils, or the angles at the base and/or the top of the nose may be altered.  Sometimes, an open technique is used on a particularly challenging nasal tip, but the incision site heals beautifully. 

Many times, internal breathing problems are corrected at the same time as the rhinoplasty is performed.  If the septum inside is deviated, blocking one’s breathing space on one side or both, it is crucial to clear the passageways of the nose before narrowing it externally to prevent any further breathing difficulty.  Moreover, many times, when noses are crooked inside, they also grow crooked outside, or they may have a hump on the dorsum (the main bridge) of the nose.  Therefore, in order to correct the external nasal deformity, it is necessary to improve the nasal base, which functions much like the foundation of a tent, where the canvas is stretched over its structure.  The portion of the surgery that may be considered medically necessary is known as a septoplasty or in some cases, a nasal reconstruction, which may be covered by one’s insurance if the policy so allows.

THE INITIAL CONSULTATION

After taking your health history, examining the inside and outside of your nose, and listening to your personal desires, Dr. Miles will offer suggestions for improving your nose, based upon a thorough evaluation of your options.  Skin type, ethnic background, age, degree of deformity, past nasal surgeries, and function of nasal structures are all important factors to be considered in creating realistic expectations.  His extensive background in ENT and Facial Plastic surgery provide you with the highest level of training and expertise.  Dr. Miles will also discuss pertinent information regarding the surgery, any possible risks or complications, type of anesthesia used, and issues that may influence the outcome of the surgery or that relate to you personally.

You can view before and after pictures of actual patients, although each  case is unique. Therefore, we provide a personal computer imaging session, which allows you to visualize possible changes to your nose before surgery takes place and to gain a better understanding of the surgery, your particular facial structure, profile, etc.  This is a great opportunity for communication between the doctor and the patient.  Usually, his trained imaging consultant will create his recommendations on the screen and discuss them with the patient; then, Dr. Miles will review the results further, make any necessary corrections, and also do real measurements with a drawing prior to surgery.

You will receive an itemized fee quote so that you may know what to expect regarding surgical, operating room, anesthesia fees, medications, and any required lab work, x-rays, EKG, etc., needed prior to the surgery.   If you wish to file any medical portion on your insurance, please bring your policy information with you and investigate beforehand your policy restrictions and coverage.

Insurance will not cover cosmetic nasal enhancement, but it may cover (in whole or in part) surgery to correct or improve nasal function.

Prior to any surgery, you will have an in-depth, preoperative visit where final plans are made.  Dr. Miles wants you to feel confident and even excited about your decision to have nasal surgery.  He believes that a good rapport with the doctor is almost as important as the surgery itself.  Your attitude and cooperation regarding follow-up care is essential to the outcome of your procedure.

THE DAY OF SURGERY

You will arrive at our private, fully-accredited operating facility early in the morning, and you should be able to return home by early afternoon. Your surgery, which usually takes only an hour or two, will be performed under a safe, local anesthetic plus IV sedation; the anesthetist will monitor you throughout the procedure and in the initial period that follows.  Your friend, spouse, or relative can sit with you in the recovery room following surgery.  The nurse will review your post care, medications, and time for your return visit with your designated person.  Then, you will be taken by wheelchair to the vehicle that will transport you home. 

If your surgery is mainly medical and not cosmetic and you prefer (or your insurance company) to have your surgery performed in a hospital, Dr. Miles is willing to do so.  However, due to the added risk of germ exposure  (where ill patient care is involved), he usually prefers to use his own accredited facility, experienced staff, anesthesia techniques, special endoscopic and laser equipment, etc.

POST SURGERY

Although discomfort is usually mild and confined to a headache that evening, you will be given oral pain and rest medication as needed.  You will experience some stuffiness, and elevating the head helps.  Dr. Miles will provide you will a special ice pack to rest on the eye area for two days to help prevent any swelling or bruising. You will have a small splint on your nose for approximately a week.  When a septoplasty for breathing improvement is performed as well, a small, internal splint may be placed inside the nose along the dividing wall of the air passages to maintain stability of the septum.  Dr. Miles does not use conventional “packing” of the nose but does use a tiny sponge and breathing tube in each nostril overnight in some cases of septoplasty.

Also, you will have something to clean and possibly irrigate the nose.  Mild nose drops are sometimes necessary, as you must be careful not to blow the nose.  The sutures are usually absorbable and do not have to be removed. 

Some activities will be prohibited in the weeks following surgery, and you must be careful not to injure the nose for 6 to 8 weeks.  If you wear glasses, special arrangements must be made to keep the glasses from resting directly on the bridge of the nose as it heals.  Tape and other devices are sometimes used to permit the wearing of glasses.

Since the nose continues to take shape over the next few months, follow-up care is vital to monitor healing.
