BREAST ENLARGEMENT / ENHANCEMENTWomen of all ages can be sensitive about the size and the appearance of their breasts. Perhaps one’s breasts never developed sufficiently or have changed over the years due to age, nursing, or gravity. Then, of course, there is the pressure that society and advertising places on women, by putting so much emphasis on a woman’s breasts as related to sensuality. If you are considering a breast enhancement yourself, you will be comforted somewhat in knowing that you are not alone. Cosmetic breast surgery continues to be one of the most popular procedures performed today on women ages 18 to 60. WHERE TO BEGIN—THE
INIT (1) If you come to Dr. William K. Miles at the Advanced Cosmetic Surgery Center, you will receive an in-depth, personal consultation with the Dr. himself (not merely a staff person). You can discuss your desires, see before and after pictures of patients, find out what is best for your particular anatomy (by taking body measurements including chest and hip size, looking at breast volume, nipple position, etc.), explore methods of implant insertion, learn about the various types of implants available, understand the risks and complications as well as advantages, and be able to plan on the recovery time, costs, etc. involved with the surgery. (2) The Dr. will give you a special “homework” sheet to guide you in deciding the size of implants you desire. Later, he will review your decision, going over the “water baggie” tests again to achieve an approximate measurement, looking at the breasts photographs that you selected to more clearly understand the size you are actually visualizing for yourself, and then guide you to a realistic expectation. (3) The patient coordinator will provide you with an itemized, written fee quote, and information on the doctor, the facility, and other services our center provides. SCHEDULING YOUR SURGERY (1) Dr. Miles performs surgery Monday through Friday in his private, fully-accredited surgical facility. We try to honor your time constraints and schedules, but if you have an exact date that you would like, it is best to schedule early. You simply call our patient coordinator, tell her a date or time frame you would like, and she will take care of the rest. (2) Within days, the coordinator will send you a surgery information packet, which details the costs, methods of payment, dates and times of your pre-operative visit and your surgery, pre and post surgical instruction sheets, lab requests, etc. (3) You will be required to send in a deposit to hold your surgery date. POST SURGERY (1) You will remain in our recovery room until you are able to go home. (2) You will be taken by wheelchair (for your safety) to the vehicle and individual who will be driving you home. (3) You will receive oral pain and rest medications as needed. Pain is moderate but certainly tolerable. (4) You will return to our office the following day to remove your bandages and be given further post op instructions. (5) In most cases, recovery time ranges from three days to one week.
MODERN ENDOSCOPIC TECHNIQUES Dr. Miles prefers the latest endoscopic techniques to augment breast size, which leave no scarring on the breasts and shortens the recovery time. With both the transaxillary (one tiny incision under each armpit) and the transumbilical (one tiny incision in the navel) approaches, Dr. Miles feels he can accomplish a nice pocket (either above or under the muscle) in which the implant seats, and more adequately increase the distance from the nipple to the inframammary crease (especially important in very small breasts), so the breasts will have a natural shape and appearance. Furthermore, he believes that the further the incision site is from the implant itself, the less chance of infection around the implant and loss of sensation to the nipples. These endoscopic procedures are made possible through advanced technology, which allows the doctor to view the breast area by means of a fibro-optic endoscope that transmits images to a video monitor. The surgery requires much smaller incisions, since the doctor is able to insert the implant through a very narrow tunnel under the skin where it is positioned and then filled with sterile saline. Note: Silicone implants are available to individuals who have previously had them, for reconstructive purposes, or for certain types of breast cases, where silicone is the preferred choice.
WHY DOES AN INDIVIDUAL SEEK A BREAST ENLARGEMENT?
For many women, having nice breasts is
inextricably connected to sensuality and femininity. And even if it
isn’t an important issue to a woman personally, she often finds herself
feeling inadequate in the “body conscious” society of today. Perhaps that explains why breast augmentation is still
one of the most popular cosmetic procedures for women, in spite of the silicone
controversy that dominated the headlines a few short years ago. Not only
has the breast implant scare subsided, but silicone implants are being
reintroduced to the American public on a limited basis. However, according to

while accurately viewing the surgical field through state-of-the-art video
equipment. He is particularly impressed with the transumbilical
approach since patients have reported less discomfort, a rapid return to normal
activities, and one barely visible incision in nature’s own scar, the navel. “All of these choices can be overwhelming,” notes
Dr. Miles, “and a careful, detailed evaluation is crucial.” He
believes that analyzing factors, such as the amount of breast tissue, the size
of the chest wall, the overall body proportions, the position of the nipples,
the amount of sagginess, and even one’s lifestyle and family history all play
a part in the decision process. “But,” Dr. Miles explains, “the
patient’s input is just as important. Therefore, I give a homework
assignment to make sure I fully understand the patient’s desires and in turn,
